Submit this form to get access to the Fitness & Swim Center as well as our digital fitness app, Mindbody.
Vehicular Gate Tag Request
Submit this form to request RFID windshield tags, which will allow your vehicle access into the community. Each household is limited to a total of two car tags and one golf cart tag per household.
Temporary Vehicular Tag Request
If you’re renting a car that you’ll be driving for 30 days or more, submit this form to request a temporary RFID tag.
Golf Club Account Activation Request
After you close on your home, submit this form so you can charge your food and beverage purchases, as well as golf fees, to your Kissing Tree Golf Club account.
Clubs & groups
Group Events & Updates
Interest groups can use this form to notify us that their club info needs to be updated on the website, tell us about upcoming events you’d like promoted, or alert us to a change in leadership or point of contact.Â
Room Reservation Request: Clubs & Groups
If you’re a Kissing Tree chartered club or group leader, use this form to request to use space at Grand Central or The Mix for club activities.
Property management
Close Patrol Request
Going on vacation or leaving your property unattended for a period of time? Submit this form to have the security team check on your house while you’re away.
Please review the program guidelines before submitting your request. The service is limited to 30 consecutive days and is intended for temporary absences only.
Design Review Application
If you’re looking to make changes to the exterior of your home, submit this application request approval from the Architectural Control Committee. You should wait to schedule vendors until after you receive written approval for your modification.
New Tenant Registration
If you’re a property owner who rents out your Kissing Tree house, you or your property management company must submit this form each time a new tenant signs a lease.

Community Concern
Report concerns or damage to Kissing Tree amenities, including community landscape, that require the attention of the Community Life team.
HOPA Age Qualification Form
Kissing Tree residents must complete this form when they move into the community to comply with the Housing for Older Persons Act.
HOPA Biennial Affidavit
Kissing Tree residents must complete this form every two years to comply with the Housing for Older Persons Act.
Facility Reservations & Rentals
KT residents may use this form to request to use space at The Mix or Grand Central. There are two types of requests that can be made through this form: reservations and private rentals.
